Health Forms & Requirements

 
District 308 requires health forms listed here be submitted to the school's nurse by September 15th to avoid exclusions from school. You can find your school nurse through the health services page or by searching your school's site directory. Any additional questions can be directed to (630)636-4361

Bus Route Information

 
To view or change your students' bus route information, you can visit the transportation page and be sure to download the bus tracking app to stay up to date on alerts and traffic delays.
