Health Forms Needed For School
Thank you to families that are working to ensure students have all necessary health paperwork by the first day of school!
Physicals and Immunizations must be submitted on or before September 15 to prevent exclusion from school. (Exclusion applies to physicals and immunizations requirements only).
Families will find the annual health history questions embedded in the electronic registration process through PowerSchool. All students complete this form annually.
Physicals & Immunizations
The physical must be completed within one year of the start of school. As long as the physical was completed after the first day of school last school year, it will be valid for the upcoming school year.
- Required Grades: PreK, K, 6, 9, and students new to Illinois schools
- 12th Grade Additional dose of Meningococcal Conjugate Vaccine: vaccine must be given on or after 16th birthday

Note: if your child will participate in school athletics consider asking the doctor for a sports physical while you are there
Sports Physical

Required for students participating in district sports, valid for 395 days instead of one year.
Notes: Sports physicals DO NOT fulfill the requirement of the health physical above. The standard health physical can suffice for both sports and state health requirements.
Please submit:
- Elementary and Jr. Highs Sports physicals submit to the school nurse.
- High School Sports Physicals submit to the athletic department
Medication Authorization
Emergency Action Forms
For any student with asthma, allergy/anaphylaxis, food allergies, seizure, diabetes or any condition that may require emergency response
Forms available on Google Drive, and your doctor can print these as well.
Dental Exam
Required Grades: K, 2, 6 and 9, Dated within 18 months of the current school year.
Eye Exam
Required Grades: K and students new to Illinois, (form dated within a year of Oct. 15).
- The school provides vision screening for grades PK, K, 2, and 8 & students with IEPs, and transfer or if recommended by a teacher. This does not count as the required eye exam for K.
- The school provides hearing screening for grades K,1,2,3, & students with IEPs, and transfer or if recommended by a teacher. There are no state requirements for outside hearing exams.
Waivers
For Students who cannot be vaccinated for medical or religious reasons
- Medical Reasons- Doctor needs to provide a note in writing the reason the immunization cannot be administered and include the duration that the waiver applies.
- Religious Reasons- Doctor must complete a religious exemption form
